We are committed to keeping your personal information safe and private. Only the people working on the project will see it. We won’t tell anyone who you are when we talk about the project. The details you fill out on this form and what you say in the group discussion will only be used in the project and you will not be individually identified. We will also ask that you respect the confidentiality of your peers. Please do not share any of what is talked about during the focus group discussion. While we will try our best to keep your information safe, we cannot promise that everyone will do the same.
Any information you share with us will be used to put together a report with ideas for how Fresno County Department of Behavioral Health can help young people avoid problems the law. You can choose to stop participating in this project at any time or request that your information not be included in the final report by contacting us at (559) 601-0153 or FresnoACEs@aurrerahealth.com.
